QUALIFICATIONS:

• Currently licensed to practice nursing in the state of NC (active in good standing).

• Must have interest and empathy with client concerns.

• Must be detailed oriented & ready to operate in a professional setting.

• Employee must be a mature, stable individual willing to work in a team environment.

DUTIES INCLUDE:

• Inputting and processing new client referrals

• Conduct admissions for incoming clients.

• Conduct client supervisory visits every 80 days.

• Plan and organize own weekly and daily schedule

• Develop and implement nursing care plans, including revision of these plans when necessary.

• Report observations/symptoms to the doctor, including changes in the patient's physical and/or emotional condition.

• Teach, supervise and counsel the patient and other family members regarding care needs and other related patient needs at home.

• Follow up with discharge procedures when a client's services become discontinued.
